Retinal vein occlusion (RVO) is a sight-threatening condition. It is the second most common retinal vascular disease after diabetic retinopathy. Central venous occlusion (CRVO) is considered the worst form, with a prevalence of almost 1 in 1000 in the population ≥ 30 years.

The purpose of the work

Quantification of changes in disc and macular vessels in patients with CRVO using optical coherence tomography angiography.

patients and methods

In our study, we included twenty eyes of twenty patients with treatment-naïve cases of CRVO. These cases were selected from the outpatient clinic of Ain Shams University Hospital between January 2020 and October 2021. Enrolled subjects were patients diagnosed with CRVO who had not previously received an injection or laser. Only patients with media opacity dense enough to sufficiently compromise image quality for reliable interpretation were excluded.

Results

Our study was able to demonstrate the agreement between quantitative macular vessel density (VD) measurement and the current fundus fluorescence angiography (FFA)-based assessment of macular ischemia. We were able to confirm a profound component of the pathological process in CRVO as suggested in the current literature and simultaneously assess it in relation to the superficial microcirculation using optical coherence tomography angiography (OCTA) and FFA. We found that optic nerve head VD parameters are a significant indicator of global retinal perfusion that could be of value in patient assessment and follow-up, especially if there are confounding factors for macular VD measurement, such as extensive edema. We found a significant correlation between FAZ size and measured macular vessel density, indicating agreement between the two ischemic parameters.

Diploma

OCTA is a new retinal imaging modality and offers a non-invasive alternative to the gold standard FFA. It visualizes microvascular changes in vascular disease with at least the same sensitivity as FFA. It has the distinct advantage of being easily replicable in poor quality and in patients requiring repeated follow-up. It can also be used in patients who are not suitable for or have a contraindication for FFA. However, OCTA is prone to inaccuracies due to inherent drawbacks such as segmentation and projection artifacts.
